Dear Fellow Pet Owner,

        My family currently consists of my wife and I, our son, and two West Highland White Terriers, Hamish Macbeth and Isobel Macbeth. Although Highlanders are very hardy they do require the occasional visit to their veterinarian.

        Having adopted these dogs at different ages, keeping up with their vaccinations, medical care, and general health records became a chore with dozens of individual sheets of paper to keep up with and file. Adding to this problem was having moved twice in four years and changing veterinarians.

        To ease this administrative burden I invented my own permanent health records for Hamish and Isobel. Now all I have to do is transcribe the results of their latest visit to their veterinarian, which by the way takes less than five minutes to accomplish. You may want to ask your veterinarian to fill-in your pet’s health record for you.

        I have not included any recommended vaccinations in this health record because many veterinarians have their own core vaccines that they administer to their patients, vaccination laws vary not only from state to state, but many times from county to county and city to city. This allows you or your veterinarian to customize this health record for your cat or dog family members.
